One of the most common procedures is breast reconstruction following breast cancer (reconstruction after mastectomy), where the breast is surgically reconstructed. Breast reconstruction with implants

A common procedure in which a silicone implant is placed under the skin or the chest muscle to restore volume.

Expander breast reconstruction

If the skin is tight following radiotherapy or a mastectomy, a temporary tissue expander is first inserted and gradually filled with saline solution to stretch the skin, before the permanent implant is surgically inserted.

Breast reconstruction using the patient’s own tissue (autologous fat)

In fat grafting, the body’s own fat is extracted from areas such as the thighs or stomach and gently injected to build up the breast or soften skin that has been damaged by radiation. This is known as autologous fat breast reconstruction and often requires several treatment sessions.

Scar revision

Surgical removal of thick or raised scars (such as keloids or hypertrophic scars), where the skin is sutured using advanced microsurgical techniques to produce a significantly thinner and more discreet scar.

The first step is a consultation with a plastic surgery specialist. You choose whether you want to book an online appointment or meet us in person at the clinic. During the appointment, you will be given medical information about risks, current waiting times and which techniques are best for you.

Before your procedure

Preparing for a safe operation

You will be given instructions regarding medication and must absolutely stop smoking (at least 4 weeks before and after the procedure), which is vital for the survival of the tissue and fat./p>

The day of the operation

Safe care at the clinic

You will be admitted to hospital. The procedure is usually carried out under general anaesthetic for your safety and comfort. Minor scar corrections can often be performed under local anaesthetic.

After the operation

Awakening and initial rest

You will rest in the recovery room. The area that has been operated on will be bandaged. In the case of major reconstructive surgery, you will often stay overnight at the clinic for observation.

Recovery & healing

Self-care and convalescence guidelines

Keep a close eye on your recovery. Avoid heavy lifting and strenuous exercise during the first few weeks. You will need to wear a specially fitted recovery bra around the clock to begin with.

Monitoring & results

Medical check-up and final assessment

A follow-up appointment to check the wound is usually scheduled after one week. A final check-up and assessment are booked once the tissue has healed and softened, usually after six to twelve months.

Results following breast reconstruction

The outcome of breast reconstruction is influenced by your body’s ability to heal, any previous radiotherapy, and the elasticity of your skin. Who is eligible for the procedure?

To undergo surgery with us, you must be in a stable state of general health. If you have been treated for breast cancer, your treatment (chemotherapy/radiotherapy) must be complete, and we often work closely with your treating oncologist to ensure that the timing of your reconstruction is optimal. Being smoke-free is an absolute, non-negotiable requirement for all reconstructive surgery to avoid tissue death (necrosis).

Complications and risks

As with all surgery, there are risks, such as bleeding and infection. With breast reconstruction using implants, there is a risk of capsular contracture (where the tissue around the implant becomes hard). With reconstruction using your own fat, there is a risk that the body will break down the fat, which can create small, harmless oil cysts (fat necrosis). With scar revision, the aim is to achieve a finer scar, but asymmetrical healing may occur. Your surgeon will go through these risks with you in detail.

Other treatments in reconstructive surgery

Breast reconstruction rarely involves focusing on just one side. To achieve good anatomical symmetry, it is very common to also perform a breast lift, a breast reduction or a minor augmentation on the other, healthy breast during a later stage of the operation. Is breast reconstruction following a mastectomy performed immediately or at a later date?

It varies. Sometimes, a primary reconstruction can be carried out at the same time as the breast is removed (immediate reconstruction). However, many patients undergo a secondary (delayed) reconstruction months or years after their cancer treatment and any radiotherapy have been fully completed.

Can you have scar revision treatment through the county council?

In some cases, if the scar causes serious functional problems (for example, if it restricts movement across a joint so that you cannot move your arm fully), scar revision may be covered by the National Health Service. If the scar is ‘merely’ cosmetically unsightly or itchy, the procedure is usually paid for privately at a clinic such as The Faculty.

What does an expander breast reconstruction involve?

It is a two-stage procedure. First, an empty silicone balloon (expander) is inserted under the skin or muscle. It is gradually filled with saline solution over several weeks at the clinic to stretch the skin. Once the skin has been stretched sufficiently, the expander is replaced with a permanent, soft silicone implant during a second operation.

Where can I read about other people’s experiences, like on a breast reconstruction blog?

There are many patient networks, forums and blogs (search, for example, for “breast reconstruction blog” or via the Breast Cancer Association) where women share their personal journeys. Bear in mind, however, that every body and every cancer treatment is unique; other people’s medical recovery may not be the same as yours.

How many operations are required for breast reconstruction using the patient’s own fat?

As the body naturally breaks down and absorbs some of the transplanted fat, it is standard practice to require two to four surgical procedures, spaced a few months apart, to build up a complete breast to the desired volume from scratch.

How does scar revision work?

The surgeon carefully cuts away the old, inelastic scar tissue and releases the surrounding skin. The wound is then sutured in several layers with extreme precision (often using dissolvable internal stitches) to relieve tension in the skin, creating the conditions for a narrower and smoother new scar.

Are the results of breast reconstruction using implants permanent?

Implants provide long-lasting volume, but modern silicone implants sometimes need to be replaced after 10–15 years due to natural wear and tear or capsular contracture. Your body will also continue to age, which may affect the symmetry of your breasts over time.
