What is hyperhidrosis?

Hyperhidrosis is the medical term for abnormally increased sweat production that exceeds the body’s need for temperature regulation. Clinically, a distinction is made between primary hyperhidrosis (which is hereditary and often localised to specific areas) and secondary hyperhidrosis (which is caused by an underlying condition or medication). The condition involves the body’s eccrine sweat glands, which overreact to nerve signals. An effective medical treatment for hyperhidrosis involves local injections of botulinum toxin into the affected area. This is a well-established medication that works by temporarily blocking the neurotransmitter (acetylcholine) between the nerve ending and the sweat gland. When the signal is blocked, the sweat gland stops producing sweat, resulting in a highly effective and targeted treatment for excessive sweating, entirely without surgery.

A successful clinical outcome always begins with a thorough consultation. Our specialists will diagnose hyperhidrosis based on your symptoms. It is important to rule out secondary hyperhidrosis; for example, if you sweat at night (nocturnal sweats) or sweat all over your body, this may be a symptom of an underlying condition that should be investigated by a GP first.

After the consultation, you have a statutory cooling-off period of at least 48 hours (which applies to aesthetic injection treatments) to allow you to decide in your own time whether you wish to give your consent.

The area is thoroughly disinfected with chlorhexidine. Anaesthesia is rarely required in the armpits.

The treatment

Safe care at the clinic

The practitioner uses a microscopically thin needle to inject small, superficial doses of botulinum toxin at intervals of approximately 1–2 centimetres across the entire sweaty area.

Self-care and convalescence guidelines

You can resume your normal daily activities straight away, but you should avoid strenuous exercise, hot baths and saunas for the first 48 hours to allow the medication time to bind to the nerve endings. You should avoid using deodorant for the first 24 hours.

If necessary, we offer a follow-up appointment after 14 days to ensure that sweating has stopped across the entire treated area.

Results following treatment

The clinical results of botulinum toxin for hyperhidrosis are very good. You will notice a gradual reduction in sweat production within 3 to 7 days, and the full effect is usually achieved after two weeks. The duration of the effect varies depending on the treatment area and your individual metabolism, but on average the effect in the underarms lasts for around 6 to 9 months.

Who can undergo the treatment?

To undergo treatment with botulinum toxin, you must be over 18 years of age. We therefore do not treat hyperhidrosis in children or teenagers under the age of 18. You must be in good general health and must not be pregnant or breastfeeding. We only treat patients with primary hyperhidrosis; conditions caused by general medical illnesses must be investigated and managed by the National Health Service.

Complications and risks

Treatment with botulinum toxin for excessive sweating is generally very safe and well-established. Temporary side effects include small bruises or slight tenderness at the injection site. Some patients experience so-called compensatory sweating (where they temporarily sweat slightly more in another part of the body), but this is uncommon in the treatment of primary hyperhidrosis. How can you stop sweating under your arms?

Superficial methods such as antiperspirants or iontophoresis rarely help with severe symptoms. Botulinum toxin injections are currently the most effective non-surgical treatment for actually blocking the sweat glands and eliminating underarm sweating for a prolonged period.

Is there a treatment for hyperhidrosis (excessive sweating)?

In the public healthcare system, anticholinergic tablets (medicines such as Ditropan) are sometimes used to treat generalised hyperhidrosis. However, these affect the whole body and often cause severe systemic side effects, such as extreme dry mouth. Can sweat glands be surgically removed?

In the past, operations were performed to cauterise the sympathetic nerves in the chest in order to stop sweaty palms, but due to the risk of severe compensatory sweating (anhidrosis in one area and excessive sweating in another), this is now very rare. Botulinum toxin is a reversible, extremely safe and gentle method compared to surgery.
