What does buccal fat removal involve?

A buccal fat removal (also known as a bichectomy) is a cosmetic surgical procedure designed to reduce the fullness of the cheeks. Beneath the cheekbones lies a specific anatomical fat pad known as the buccal fat pad. The size of this pad is often genetically determined and is rarely affected by general weight loss. Through buccal fat pad removal, a tailored amount of this buccal fat is removed to create a more defined transition between the cheekbone and the jawline.

Our methods for buccal fat removal

We use tried-and-tested, safe surgical techniques to ensure the highest level of patient safety. Our plastic surgeons carefully assess which type of surgery, rather than liposuction, is the most medically appropriate option for your anatomical conditions in order to achieve a proportionate and natural result.

Bichectomy (Buccal fat removal)

A small incision is made on the inside of the cheek, inside the mouth. Through this incision, the surgeon can carefully identify and remove the correct amount of your buccal fat pad. As the incision is made inside the mouth, there will be no visible scars on your face.

Cheek liposuction

In some cases, the fullness is not confined to the Buckala fat pad, but is due to subcutaneous fat. In such cases, traditional liposuction of the lower cheeks or jawline may be considered.

Gentle technology and medical precision

Whether we are removing bulging fat or performing traditional liposuction, our plastic surgeons use microsurgical instruments that minimise trauma to the delicate facial tissue. A gentle and precise approach is crucial for reducing bleeding and swelling after the procedure. This promotes a safer healing process and is key to ensuring extremely discreet, or as in the case of a bichectomy, completely invisible scars.

The Evolution of You

Bichectomy
Step by step

To ensure you feel confident and well prepared, we have a clear and reliable process in place, from your first visit right through to your final follow-up appointment. Here, we provide an overview of how a bichectomy is carried out at our clinic, from start to finish.

Consultation

Medical assessment and choice of method

We will assess the structure of your face and discuss whether a buccal fat removal is appropriate, or whether the fullness is due to superficial fat in the face.

Before your procedure

Preparing for a safe operation

Once you have had time to consider it, the operation will be scheduled. It is an absolute medical requirement that you abstain completely from alcohol and smoking for four weeks before and after the procedure.

The day of the operation

Safe care at the clinic

You will be admitted to hospital. The procedure usually takes less than an hour and can be carried out under local anaesthetic with sedation, or under light general anaesthetic. The incisions are made on the inside of the cheek.

After the operation

Awakening and initial rest

You will rest for a short while at the clinic before you can go home. The stitches in your mouth are usually the self-dissolving type.

Recovery & healing

Self-care and convalescence guidelines

For the first few days, a liquid or very soft diet is recommended. You will be given instructions on how to rinse your mouth to keep the incisions clean. Avoid exercise that raises your heart rate for 2–3 weeks.

Monitoring & results

Medical check-up and final assessment

We’ll book a follow-up appointment in a week or so to make sure your mouth is healing well. A final check-up with your surgeon will take place in six months’ time.

Results of a buccal fat pad removal

Recovery following buccal fat removal surgery is usually relatively straightforward, but requires patience. During the first few weeks, it is normal for the cheeks to actually look a little fuller due to swelling inside the mouth. This swelling will gradually subside. However, it often takes between three and six months for the tissue to tighten completely and for the final anatomical result of your buccal fat removal to be fully assessed.

Our plastic surgeons specialising in facial surgery

At The Faculty, we have assembled a hand-picked team of plastic surgeons, with every cosmetic procedure being performed by a specialist surgeon with extensive expertise – the cornerstone of your safety.

Who is eligible for a buccal fat removal procedure?

To undergo surgery with us, you must be over 18 and in good general health. A thorough medical assessment is carried out during the consultation. It is important to understand that buccal fat removal is not suitable for everyone. If you already have a narrow face, or little fat in your face, removing this pad can lead to a sunken appearance as you get older and your face naturally loses volume. Quitting smoking is a non-negotiable requirement.

Complications and risks

As with any surgical procedure, there are risks involved, such as bleeding, infection (hence the need for strict oral hygiene) or asymmetry. With specific procedures inside the cheek, there is a small risk of temporary impact on the salivary duct or the small nerves that control facial expressions. By choosing an experienced specialist surgeon and an extremely gentle surgical technique, these risks are significantly minimised.

Other treatments

For patients whose fullness is due to subcutaneous fat rather than the buccal fat pad, we can recommend facial liposuction on specific areas such as the jawline or under the chin. We also receive enquiries about ‘fat-dissolving injections’. Belkyra is an approved medicine for dissolving fat under the chin, but is not used for the deep buccal fat pad inside the cheek.

Why consider a buccal fat removal?

Questions such as “how can I make my cheeks smaller?” or “how can I remove fat from my face?” are very common. The reasons for wanting to undergo cheek fat removal (face fat removal) are highly personal. Many people feel that their face looks round regardless of their body weight and wish for a sharper anatomical contour. The motivation behind seeking surgery is entirely your own, and we listen carefully to your reasons.

Frequently asked questions about buccal fat removal

Here we have compiled some frequently asked questions about facial fat and buccal fat removal

How do you remove fat from your face / how do you get rid of fat on your face?

A very common question is how to remove fat from the face or how to get rid of facial fat in general. From a purely physiological point of view, it is not possible to target specific areas for fat loss. If the fullness is due to your genetic buccal fat pad, this fullness will rarely disappear through diet or exercise, and surgery is then the only method for permanently removing fat from the face in that specific area.

How can you lose facial fat without surgery (lose fat from your face)?

If you have a higher overall body fat percentage, losing weight in general can make your face look slimmer. However, you cannot choose to lose fat from your face first, as the body burns fat systematically.

What is the difference between buccal fat removal and cheek liposuction?

This is an important medical distinction. Cheek liposuction involves using a cannula to remove the superficial subcutaneous fat located directly beneath the skin. Buccal fat removal (bichectomy) involves the surgeon entering the mouth to remove a deep-seated, encapsulated fat pad that cannot be removed using traditional liposuction.

Do fat-dissolving injections work in Sweden for this?

No. Injections that destroy fat cells (such as Belkyra) are approved and effective for smaller, superficial fat deposits such as a double chin. They are not used inside the cheek to remove the deep-seated Buckala fat pad, as the risk of damaging nerves and tissue is too great.

Can you look older after having your buccal fat pads removed?

The face naturally loses volume and subcutaneous fat as we age. If the buccal fat pad is removed from a patient who already has a fairly narrow face, there is a risk that the cheeks may appear too sunken in the future, which can give an aged appearance. This is why the medical consultation is extremely important; we only perform the procedure on patients whose anatomy is deemed to be suitable.

Is a buccal fat pad removal performed under local anaesthetic or general anaesthetic?

As the procedure is relatively quick and the incisions are made inside the mouth, the operation is carried out very smoothly under local anaesthetic combined with sedatives and painkillers. For patients who feel particularly anxious, the procedure can also be performed under light general anaesthetic for maximum comfort.

How long does it take to recover?

As no incisions are made on the outside of the face, you won’t need to treat any external wounds. The internal incisions heal quickly, usually within about one to two weeks. Your cheeks will be swollen initially, but most people can return to non-physical work after just a few days, provided you maintain good oral hygiene and rinse your mouth as instructed by the surgeon after eating.
