What does a gynaecomastia operation involve?

Gynecomastia, or ‘gyno’ for short, refers to the benign growth of breast tissue in men. Gynecomastia surgery is a procedure designed to reduce this tissue, often in combination with the removal of excess fat, to create a firmer and smoother contour across the chest. During your consultation, an anatomical assessment will be carried out to determine whether the volume is due to fat (lipomastia), actual breast tissue, or a combination of both, which will determine which method is right for you.

Liposuction

Gland removal

In cases of true gynaecomastia, where the mammary gland itself is enlarged, the tissue needs to be removed surgically. The incision is then made discreetly at the lower edge of the areola.

Gynecomastia consultation

The first step is a consultation with a plastic surgery specialist. You choose whether you want to book an online appointment or meet us in person at the clinic. During the appointment, you will be given medical information about risks, current waiting times and which techniques are best for you.

Consultation

Medical assessment and choice of method

You will meet with a plastic surgeon who will assess your suitability. We will discuss whether liposuction alone or a combination with gland removal is required.

Before your procedure

Preparing for a safe operation

Once you have had time to think it over, you will receive detailed instructions from our coordinator. You must refrain from smoking and drinking alcohol for four weeks beforehand. On the evening before the operation and on the morning of the procedure, you must wash thoroughly using an antiseptic chlorhexidine sponge.

The day of the operation

Safe care at the clinic

At the clinic, you will check in and change into a hospital gown. You will meet your plastic surgeon for photographs and skin markings, and you will have a chat with your anaesthetist. The procedure is carried out under general anaesthetic and usually takes two to three hours.

After the operation

Awakening and initial rest

You will be resting in our recovery room. It is normal to experience tenderness and swelling. You will be given pain relief and instructions on how to manage this during the first 24 hours.

Recovery & healing

Self-care and convalescence guidelines

For the first few weeks, you will wear a compression vest around the clock. Most people are ready to return to non-physical work after about two weeks.

Monitoring & results

Medical check-up and final assessment

Around one to two weeks after the operation, you will see one of our nurses for a check-up on the incisions and the healing process. A final examination and follow-up photographs with your plastic surgeon will take place after approximately six to nine months.

Results following surgery for gynaecomastia

The outcome following surgical removal of breast tissue is largely influenced by your original anatomy, the amount of tissue removed and the natural elasticity of your skin. In the early stages, it is normal for the chest to be swollen and feel tight. The skin may initially feel uneven before the swelling subsides and the tissue gradually adapts to its new, firmer shape. We closely monitor your individual healing process and evaluate the anatomical result with you after approximately six to twelve months, once the tissue has softened completely, the skin has tightened and any scars have begun to fade.

Who is eligible for gynaecomastia surgery?

How do I know if my breast enlargement is gynaecomastia or just lipomastia?

The simplest way to find out is through a clinical examination. Medical gynaecomastia consists of firm, often rubbery glandular tissue just behind the nipple. If the surrounding tissue is completely soft, it is usually a case of lipomastia, i.e. a pure accumulation of fat. Often it is a combination of both, which requires us to both suction out the fat and surgically remove the glandular tissue to achieve a smooth contour.

Is it possible to get rid of man boobs through exercise?

If the increase in volume consists solely of superficial fat (lipomastia), weight loss and exercise may, in some cases, reduce the size. However, it is never possible to exercise away an enlarged mammary gland (gynaecomastia), as glandular tissue is not affected by calorie burning or chest exercises. The gland will remain on top of the muscle, which is why many people opt for surgery.

What does the expression "chipstuttar" mean?

"Chipstuttar" is a common slang term that generally refers to breast growth in men resulting solely from an increase in subcutaneous fat, i.e. lipomastia. It differs from true gynaecomastia in that it lacks the firm mammary gland tissue. Regardless of which term is used, the approach to treating the condition is similar, with liposuction being the primary surgical method.

Is it possible to have a swollen breast gland without it being cancer?

Yes, in the vast majority of cases, a swollen breast in a man is completely benign and is due to gynaecomastia. It can be caused by hormonal fluctuations, medication or genetic factors. Male breast cancer is very rare. However, it is always important to see a doctor if you feel a new, hard lump in the breast, if the swelling is on one side only, or if the skin changes, in order to rule out more serious conditions before considering plastic surgery.

Will there be visible scars after a gynaecomastia operation?

The aim is always to leave as few and as inconspicuous scars as possible. In liposuction, the incisions are only a few millimetres long. If we need to remove a mammary gland, the incision is made at the colour transition at the lower part of the areola, which acts as excellent natural camouflage. The scars are initially red but gradually fade over the first year until they often become very difficult to detect.

How long do I need to wear a compression vest after the operation?

Compression is a very important part of your recovery. It helps to reduce swelling, prevents fluid build-up and supports the skin as it tightens around the new breast contour. The compression garment should normally be worn around the clock for about three to four weeks. Your surgeon will give you personalised advice on exactly how long you need to wear the compression garment, based on your healing process.

Is there a risk of losing sensation in the nipples?

It is very common to experience numbness, tingling or hypersensitivity in the nipple in the initial period following the operation, as small sensory nerves in the skin are affected by the procedure. For the vast majority of patients, this effect is temporary, and normal sensation returns gradually over the next six to twelve months. We always use gentle techniques to minimise unnecessary nerve damage.
